What to bring

Arrive ten minutes early the first time. If you completed the intake form at home, check-in takes about five minutes.

  • Photo ID

    A driver's licence or any government photo identification.

  • Insurance card

    Bring the card itself, not a photo, at the first visit.

  • Medication list

    Everything you take, including supplements, with doses.

  • Previous clinic

    The name and city so we can request your records.

  • Immunization record

    For children especially, if you have a copy.

  • Your questions

    Written down. It is the single best thing you can bring.

Forms and paperwork

Four documents cover almost everything. None of them need printing.

  • New patient intake

    Medical history, medications, allergies and family history. Arrives by text when you book, takes about eight minutes.

  • Records release

    Authorises your previous clinic to send your chart to us. Signed at registration or through the portal.

  • School and sports forms

    Bring the form to the appointment and it is signed before you leave, rather than posted to you later.

  • Notice of privacy practices

    How your health information is used and shared, acknowledged once at registration.

Patient portal

Results, refills and messages in one place

Your portal account is created at registration and the invitation arrives by email. It is where results land with a note from your provider, where refill requests go, and where you can message your care team without sitting on hold.

This is a demo website, so there is no live portal to sign into. On a real build this button would link to your practice management system.

  • Results with a plain-language note, including the normal ones
  • Refills requested in a few taps, usually done within two business days
  • Messages answered by your own care team within one business day, never billed
  • Appointments requested, confirmed and rescheduled without a phone call

How a visit runs

  1. Request an appointment

    Book online in about two minutes or call the front desk. We will confirm by text the same business day.

  2. Fill in your intake

    A short form arrives by text. Doing it at home keeps check-in to about five minutes.

  3. See your provider

    Vitals with a medical assistant, then 20 to 40 minutes with the provider you chose.

  4. Labs in the building

    If you need blood work, it happens in the same visit. Most results come back the same day.

  5. Get the plan in writing

    Your summary, prescriptions and next steps go to the portal before you reach the car park.

  6. Message between visits

    Portal questions are answered within one business day by your own care team.

Insurance and billing questions

We accept most major commercial plans in Washington, Apple Health, Medicare Part B and TRICARE West. The full sample list is on the new patient page. Always check with your plan, because networks change.

We publish sample self-pay rates for every visit type and charge them at the time of service. A standard visit is $110 (sample). Payment plans are available for larger balances.

Most plans cover a preventive annual visit in full. If a new problem is addressed during that visit, part of the appointment may be billed as a separate office visit. We will tell you when that is about to happen.

No. Portal messages answered by your care team are not billed. If a message turns into something that needs a full visit, we will offer you an appointment rather than bill for the message.

Request one through the portal or by phone and we will send it within two business days. Our billing team works in the building and will walk through any line you do not recognise.
